Dual-Purpose ID Card: Members will use their Florida Blue Medicare HMO or PPO medical ID card, which also serves as their dental ID card.
Plan Type Reference: The Medicare Advantage (MA) ID card will indicate the medical plan type, which differs from the dental plan type.
Dental Benefits: All dental benefits included in MA plans are PPO or EPO.
EPO Plan Limitation: Exclusive Provider Organization (EPO) plans do not offer out-of-network benefits to members.
Network Participation: Only the dental plan type is relevant to your network participation status.
