Oral Health for Overall Health
If you’ve been diagnosed with an eligible medical condition and need to enroll in Oral Health for Overall HealthSM, simply fill out the form below. If you have Florida Blue medical and dental plans, you may have been enrolled automatically. If you’re already enrolled in the program, learn how you can improve your overall health.
Eligible medical conditions include diabetes, coronary artery disease, stroke, oral cancer, Sjögren’s syndrome, pregnancy, head and neck cancers, COPD, end-stage renal disease, and metabolic syndrome.
You can also print and mail a paper form to enroll.
